Check the Review History of a document by clicking the Views/Edits/Redcations tab when viewing a document.

You will be able to see the Review History of when Responsive Issues, Privilege and Confidentiality Status has been changed, the Redaction History, when Modifications have been made to coding data, along with Views, showing who has looked at the document and when.
